"Can I have two specialties?"
Absolutely. You can have as many specialties as you like.
If you are asking me about marketing your practice, I suggest you only market one speciality at a time. You will appear more competent to your prospects. Let me show you.
If I tell you I specialize in working with adults with A.D.D, children of divorce, and women in mid-life, would you say "Wow! I want to send you a referral?" My guess is that you would see me as all over the map. My name might not come to your mind when you want to make a referral.
Remember, treating a specialty and marketing a specialty are different issues. Of course you will treat many clinical problems in your private practice. If you want to fill your practice more consistently over time, you will want to have a reputation as helping relieve a certain type of pain.
My advice is that you create a plan to market one specialty at a time. It will make the most of your marketing time and dollar. As you grow and change, so will your choice of specialties.
Pick something you enjoy. Then tell people about it!
Thanks for a great question!
